PCMH Coach for the state of Tennessee
Community Care of WNC (CCWNC) is seeking a PCMH Coach to support our engagement with the Tennessee Division of Health Care Finance and Administration by overseeing and providing training, technical assistance services and on-site coaching to Tennessee primary care provider practices transitioning to patient-centered medical homes.
The PCMH Coach will provide direct primary care physician practice support, including the conducting of initial assessments, onsite coaching and semi-annual assessments for provider practice sites. He/she will effectively train practices and equip them with best practices and approaches to optimize their population health impact, control total cost of care, and improve the patient experience. He/she will support the program’s quality performance objectives through utilization of established quality improvement methodologies, QI tools and techniques and team facilitation activities.
Required qualifications for the PCMH Coach include: two or more years of relevant experience; demonstrated expertise in quality improvement, patient engagement, and general practice transformation; and a strong knowledge of primary care transformation processes and Tennessee’s PCMH and Health Link program. A background in primary care service delivery and/or care coordination preferred. NCQA PCMH Certified Content Expert a plus. At least two years of work experience with rapid-cycle change, practice facilitation and data analysis preferred. A minimum of an Associate’s degree in Business Administration, Organizational Development, Nursing or other health related field required.
This position is home-based and requires that the PCMH Coach has a residence in the state of Tennessee - Nashville area. Travel within the state of Tennessee is required.
